MY ACCOUNT

One of the most important services you have is My Account, your secure online portal where you can view your plan coverage, download your ID Card, view your claims information, view any messages sent to you, and find many other helpful resources. If your school requires you to provide a pdf of the ID card then you will need to set up a My Account to obtain a copy of the card.  My Accounts can be set up approximately 72 hours after your enrollment is complete.  Please Note ID cards for a policy are not available before June 15th. If you have not created an account, learn more here.

HEALTHCARE PROVIDER SEARCH

Look for a specific provider by name and/or request a list of providers within a certain mile radius from your school.

BEHAVIORAL HEALTH PROVIDER SEARCH

Search by name or by mile radius from your school within the provider network for a psychologist/psychiatrist

PROVIDER CLAIMS

Claims should be submitted within 90 days of the date of service.

Three options for claims submission:

  1. Electronic – Provider submits electronically – Payer ID #74227 (student does not need to submit claim form with this option)
  2. Email – A scanned copy of the completed form submitted by provider or student to SI.DRG@uhcsr.com
  3. Hard Copy Submission – Provider or Student may mail to:

PERSONAL REPRESENTATIVE APPOINTMENT

To authorize another individual to act on your behalf for health insurance matters with UnitedHealthcare StudentResources, simply log into My Account and complete the Personal Representative Appointment Form (PRA Form) in the Forms & Letters section of the home screen. Alternatively, you may download here the PRA Form, complete, and fax it to the number indicated on the form.
